Transition Services for Schools

To assist with transitioning from school to adult settings Onemda has developed a range of transitions services, including:

  • Transition Services Image Options

Onemda in partnership with local schools offers a suite of programs which familiarises school leavers with post school options.

  • Pre-Planning

Onemda can provide pre-planning assistance to help develop an NDIS plan identifying the services and supports that will best meet the needs of the person with a disability and how to access the service system.
